Transition & Independent
Living Support

Building the skills, connections, and plan every resident needs to move from our housing program into stable, independent living — with confidence and community behind them.

Preparing Residents for
Life Beyond Our Doors

Our Supportive Recovery Housing program is temporary — up to 180 days. That is by design. The goal is not to create dependency on our housing, but to use that time to build the skills, connections, and stability that make independent living possible.

Transition planning begins from day one. We work alongside each resident to understand their long-term goals — permanent housing, employment, community connection — and build a realistic path toward them.

No resident leaves our housing without a transition plan and, wherever possible, warm connections to the next support they will need.

From Day One to
Independence

Transition support isn't something that happens in the last week. It begins when a resident arrives and runs through every stage of their housing program.

1

Goals Assessment

On arrival, we understand your long-term goals and what you're working toward after housing.

2

Skills Building

Throughout your stay, we build practical life skills alongside your recovery progress.

3

Transition Planning

Well before your exit, we build a personalized plan covering housing, income, and ongoing support.

4

Warm Handoff

You leave with real connections — to housing, support, employment, and community — not just a list of phone numbers.
